Triad of Prostate Symptoms

A revolving door of inflammation, pain, and congestion can occur in some men with prostatitis, pelvic tension disorder, and prostatic congestion which often feels like a downwards spiral with no way out.  This occurs when one of these conditions is present and causes another condition to occur.  Let’s use a man who gets a bacterial infection in the prostate as an example.  As the infection sets in, the prostatitis becomes inflamed causing pain.  The pain leads a subconscious tensing of his pelvic muscles as a self-preservation reflex.  This leads to pelvic tension which inhibits his ability to empty the prostate adequately during ejaculation.  This backup of prostatic fluid causes prostatic congestion which further inflames the prostate leading to more prostatitis.  Then the cycle continues and can spiral downwards.

For many men, they will need to focus on all three conditions at once and if anything, try to move the cycle in reverse.  Less pain can lead to less tension, which can lead to less congestion, which can lead to less pain, etc.  This often requires patience and time in order to reverse these cycles, especially if the cycle has been going on for some time.  In many cases, recovery is two steps forward and one step back.  However, a prostatitis flare, musculoskeletal injury, sexual stimulation without ejaculation (failed wet dream, sexual denial, or viewing pornography) may actually push the cycle in the wrong direction and require some backpedaling.  Knowing your body and what works for you can help you and your healthcare provider come up with a plan that works best for you individually.  Knowing what medications and non-drug therapies are at your disposal can be a huge help.  If symptoms persist or worsen, always reach out to a healthcare professional, especially ones trained in these disorders.

Sexually inactive men in particular can be quite symptomatic when the individual has all three conditions occurring simultaneously.  When the prostate is congested and full, the body may attempt to have a wet dream.  Pain and sensitivity can cause the individual to wake before ejaculation completes on its own, leaving the individual highly aroused and congested.  This may often occur after there has been a fair amount of sexual stimulation during sleep and more seminal fluid has been produced.  When ejaculation does not occur, this leads to a worsening of congestion which can increase the number of attempts a man’s body may make to have a wet dream, often waking him multiple times a night.  This can lead to sleep deprivation, stress, and in some cases a tremendous build-up of seminal fluid that hasn’t been released.  In our survey we asked if symptoms of congestion led the individual to masturbate to relieve the congestion.  Here are the results:

In men who do not masturbate for moral or religious reasons, this can also be highly distressing emotionally since the temptation to masturbate to complete the process is typically extremely high.  If repeated failed wet dream attempts and severe congestion ultimately results in the individual giving in to that temptation and masturbating,  although they have strong personal or religious beliefs about not masturbating, the result can be emotionally and/or spiritually devastating in some cases.

Men with religious or personal views against masturbation should not infer from what has been said here that men should masturbate to finish off failed wet dreams.  Speaking with a religious counselor or leader after seeking treatment for underlying concerns can be very beneficial.  In some cases, a religious counselor or leader may approve  masturbation for strictly medical reasons or have other counsel.  Having the input of trusted religious leaders can often eliminate second guessing and guilt; however, gaining personal confirmation on sensitive matters such as these is prudent.

In order to assist individuals who want to increase the chances of having a completed wet dream naturally and potentially reduce the temptation to masturbate, the following suggestions may be helpful: 

  • Taking zinc before bedtime to help decrease prostate secretions and increase testosterone levels to increase the chances of having a wet dream.  It also makes it possible for the body to recycle seminal fluid better thus reducing congestion.  Taking vitamin D (1,000u per day) may also slightly increase testosterone levels increasing nocturnal erections that can lead to successful wet dreams.

  • Taking a hot bath before bed can help thin out old congested seminal fluid and relax the pelvic floor muscles thus making wet dreams easier to complete rather than having the male body reach a high state of arousal without being able to ejaculate

  • Doing some upper leg/groin stretching to relax muscles before falling asleep can also help relax the muscles and reduce tension that can halt a wet dream process. 

  • Sleeping in can help since many men have wet dreams in the early morning hours as it will give the body a greater chance to sleep long enough to ejaculate. 

  • If pain is a culprit for waking an individual up from the wet dream process before ejaculation occurs automatically, taking a pain reliever like ibuprofen or naproxen before bedtime can decrease pain and potentially improve the chances of sleeping long enough for a wet dream to complete in ejaculation naturally.

  • Upon waking up without ejaculating, let an erection subside enough to go urinate and empty the bladder.  Pressure from a full bladder can increase the sexual sensitivity of the prostate which can be a main driving force in waking men up too early in the wet dream process and thus preventing ejaculation naturally.  This may result in the individual completing a wet dream after he goes back to sleep with an empty bladder.

  • Don't get emotionally worked up about something that is really outside one's control.  For this reason, many men find it very helpful to simply talk out their frustrations or difficulties with a friend, family member, religious leader, or healthcare professional that can have an outside perspective and offer reassurance and possible ideas on how to move forward.  Being able to talk openly, and in detail without embarrassment or reservation can help individuals evaluate exactly what’s going on rather than becoming more and more frustrated. Oftentimes this can result in a visit to a healthcare provider to get valid treatment for prostate issues that may be causing the failed wet dreams.

  • Getting treatment for prostatitis, pelvic tension disorder, and/or prostatic congestion.
